Residency Requirements

An individual who wishes to participate in a sport at CHS must meet certain residency requirements:

Live in the Portland Public School District (PPSD)

Reside within the attendance boundaries of CHS or, according to PPSD guidelines, be accepted as a transfer student to CHS

   

Academic Requirements

An individual who wishes to participate in a sport at CHS must meet certain academic requirements.

At the end of each grading period, Cleveland's Athletic Director provides each coach with a list that includes the GPAs of each athlete on his/her team. The coach then uses that information to monitor the academic performance of each athlete.

* Students on probation must participate in one-hour tutorial sessions a minimum of three days a week.
** Once ineligible, an athlete cannot become eligible again until the end of the next grading period. Appeals may be made to the PPSD Athletic Committee following the existing hardship process.

Freshmen Only

All Freshmen, regardless of their final grades from the previous semester, are eligible to participate in a sport during the fall season at CHS. However, for every grading period thereafter, they are subject to the academic eligibility standards presented in the tables above.

Academic Mentorship Program

During the first grading period of the 2008-09 school year, Cleveland initiated an Academic Mentorship Program for all freshman athletes. For details, view the letter to parents/guardians from Cleveland's Athletics Director (PDF).

Medical Requirements


Physical Exams

All students who participate in practice and/or competition in a CHS athletic program must be examined by a physician possessing an unrestricted license to practice medicine, a licensed physician assistant, a certified nurse practitioner or a licensed chiropractic physician.

A physical exam is required every two years during a student's high school career.

A Physical Clearance Form documenting the athlete's ability to safely participate in a CHS sport must be signed by the examining medical professional and submitted to the CHS Athletic Director before the athlete may participate in a CHS sport. School

During the alternate years when a physical exam is not required, the athlete must complete and submit an Update Clearance Form.

Both the Physical Clearance Form and the Update Clearance Form are available:

Physicals for incoming freshmen that are completed after spring break during the students' eighth-grade year are valid for two years.

(NOTE: Cleveland's School-Based Health Center performs physicals. That office can be reached at 503-988-3350.)

 
Medical Card

Each athlete must also provide a completed 3"x5" emergency medical information card (sample only, PDF, 2,775KB), which is available in the CHS Office or from the CHS Athletic Director. It lists the emergency medical and insurance information that pertains to each athlete. It also authorizes a coach to act on behalf of a parent who is unavailable in the event his/her child is injured during a practice or competition.
